♔Will someone be a doll and make me a few form?♔

    So I need a neat, creative, and colourful form. If you can I need a couple, here's what I need them for



    YewDale School for the pregnant-exactly like the title I need a form for this one. Needs to have three forms-one for the mother, one for the father, and one for the child. If you don't mind. The ones for the mother and father must have places to put lots of pictures, for the kid they need one or two or more if you don't mind.



    The Powerfuls-they have special powers if you couldn't tell. I need somewhere you can put what position they are, needs a lot of pictures on the form,


    Remember must be advanced forms
